How do I make an appointment?
All appointments must be made online via this link:
We are not "in-network" providers with any insurance policies, however, if your insurance plan offers "out-of-network benefits", we are happy to provide you a receipt of services so you can submit it to them with a claim form for direct reimbursement. Payment for all services and programs are due at start of service.

What forms of payment do you accept?
We accept Cash, Check, Visa, Mastercard, Discover, and American Express as well as HSA cards with a Visa or Mastercard logo. There maybe a processing fee associated with credit card transactions.
Do you Accept Workman's Comp or Personal Injury Cases?
We do not accept workman's comp or personal injury cases. However, if you had a previous work or auto injury, are still experiencing persistent symptoms, and are not currently in litigation, we are happy to help you in your care.
